How do I know if a P.E.A.K. program is right for me?

Do I need a referral to be seen at P.E.A.K.?

The state of Maryland has Direct Access for physical therapy.  This means a physician's referral is NOT required to be treated by a licensed physical therapist.  If we determine that the attention of a medical doctor is needed, we will refer you to your primary medical doctor for follow up.

What can I expect at my first session?

Dr. Fish will review your medical history with you and discuss your current symptoms and goals.  She will then complete a detailed examination of your posture, alignment, strength, range of motion, sensation, balance, coordination, and gait.  A personalized, comprehensive program will be designed just for you!

How can I prepare for my first appointment?

  • Please bring your ID as well as any completed new patient paperwork if not done through your patient portal
  • Please bring any imaging reports from a radiologist
  • Please wear loose, comfortable clothing and footwear that are appropriate for exercising and allows for easy access to the treatment area

How many sessions will I need to achieve my goals?

The number of appointments necessary to achieve your goals varies based on condition.  After your initial evaluation, Dr. Fish can suggest a recommended number of sessions which typically ranges from 6 to 10 over a period of 4-6 weeks.

How much does each session cost?

Our services range in price.  Please contact P.E.A.K. for pricing.  Payment is required at or before the time of service.  Our payment is based on 30-90 minute Telehealth or In-Person sessions entirely with a Doctor of Physical Therapy. 

Is P.E.A.K. Physical Therapy and Performance Training now accepting Medicare patients?

We are happy to say that we ARE now accepting Medicare patients.   We see patients under Medicare Part B in the comfort of their own home.  We also continue to see many of these patients who eventually transition to fitness and wellness services after their initial plan of care has ended.  This means that we can continue to work with you to improve your fitness to safely perform your activities of daily living and to prevent injury.  

Why does P.E.A.K. Physical Therapy and Performance Training not accept commercial insurance?

P.E.A.K. is an Out-of-Network provider with most commercial health insurance companies.  This allows us to provide high-quality one-on-one care that is personalized to your specific condition and goals.  We are not restricted by insurance regulations and can decide what care is best for you.  Depending on your benefits, you can submit to your insurance company Out-of-Network therapy benefits.  Forms are provided for you by your insurance company to be filled out and sent in along with needed receipts and treatment codes which can be provided for you by P.E.A.K. following your session.  Your reimbursement rate is set by your insurance company.  Please call your provider before you visit P.E.A.K. to confirm out-of-network therapy benefits as well as any pre-authorization requirements.  You will never receive any unexpected bill from P.E.A.K.

What is wrong with traditional in-network physical therapy in outpatient and home settings?

Nothing is wrong with insurance-based physical therapy.  However, health insurance companies continue to cut reimbursement rates for physical therapy services and strictly regulate how therapists are able to treat patients.  Therefore, most outpatient clinics treat multiple patients each hour, rotating each patient between an aid or a tech for the majority of the session.  Patients in this model often do not receive the consistent, skilled, one-on-one care with a professional that they require and, as a result, stay in physical therapy longer than if they had received high quality care the entire time.  If you are looking to work directly with a Doctor of Physical Therapy to receive the best care that is personalized to your specific needs, you have come to the right place.

How much will I be reimbursed from my insurance company?

Your reimbursement depends on your particular plan and your specific insurance company.  Most people do receive some financial reimbursement.

What is your no show/cancellation/late arrival policy?

We understand that sometimes in life your plans can change.  Please let us know 24 hours ahead of time before your appointment and no cancellation fee will be applied.  If you cancel for any reason less than 24 hours before your appointment or you simply do not show up, then a cancellation fee of 50% of the visit cost will be charged.  If you are running late, please call to let us know.  However, if you arrive late to your visit, your session fee may be prorated based on any remaining therapy time.  


We respect and place a high value on your time as a patient and ask that you show the same respect for us as well as subsequent patients.

Good Faith Estimate

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost


Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.


  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
  • Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
  • Make sure to save a copy or picture of your Good Faith Estimate.
